On the 25th May 2018 a law was passed to ensure that your data is safe and subsequently means that companies must be transparent with you in how they store and use your information.  

To comply with the General Data Protection Regulations, our privacy statement is listed below for your information.  

 

What? 

  • When you book an appointment, we collect your name, address, contact number, email address and how you heard about us. 

  • When you have any type of consultation within the practice information about your overall health, lifestyle and treatment plans will be stored for future reference. 

  • Clinical notes will be written at the end of every appointment, included will be details of problems reported, treatment/resolutions given and plan of action for future visits.  

 

Where? 

  • We use two protected systems to store data, one system will hold all your personal details and clinical record and the other will hold your hearing loss details and hearing aid prescription.  

  • Both systems are password protected and can only be accessed by Sarah-Jane Brown. 

 

Why? 

  • We use your information so that we can identify you.  

  • To contact you for appointment reminders and to keep you up to date with the latest clinic information. 

  • We keep your medical information and treatment details to ensure that you receive a high & consistent level of care. 

 

We promise that we will never sell or share any of your personal information with any other provider.
